We offer transparent, easy-to-understand insurance and self-pay options. All fees are due at the time of service. Pricing may vary based on individualized treatment plans, product amounts, and specific medical needs.

If you plan to use your insurance, please:

1.Bring your insurance card to your first visit

2.Verify your benefits and coverage for our services, as some plans may require prior authorization or have limitations.

3.Be aware that copays, coinsurance, and deductibles are determined by your individual insurance plan and are due at the time of service
 

Out-of-Network Insurance

If your insurance is not accepted by Luminous you will be responsible for payment at the time of service. We can provide a superbill for you to submit to your insurer for possible reimbursement (reimbursement amounts vary based on your plans out-of-network benefits. Please check with your insurance provider for detailed information).
 

Cash Pay/Self Pay Options

For clients who do not have insurance or prefer not to use insurance, we offer simple and transparent self-pay rates.

Cash pay service fees are due at the time of service.

We accept the following forms of payment:

• HSA/FSA cards

• Cash

• Check (returned checks are subject to a $30 fee)
